Grown-Up Swimming Media Release Form

  1. I, the undersigned, hereby authorize Grown-Up Swimming and U.S. Masters Swimming, their related entities, agents, directors, partners, contractors, volunteers, consultants, employees, successors, and assigns of Grown-Up Swimming, U.S. Masters Swimming and their related entities (“GUS”), the irrevocable right to photograph, take motion pictures or video footage, make electronic sound recordings, conduct recorded interviews with or without name, for any purpose, including publicity, publication, illustration, advertising, promotion, trade, educational and other public media in perpetuity throughout the world, and without restriction as to alteration (the “Media”) of either me or the Minor/Ward, as defined below (together, me, I, and/or the Minor/Ward may be referred to herein as the “Participant”). 

  2. I understand that I, and/or Minor/Ward, may be identifiable from such Media. I acknowledge and agree that neither I nor Minor/Ward, have the right, title, or interest in the Media and waive all rights to any compensation, and further acknowledge that the copyright therein is the exclusive property of GUS. I, individually, and/or on behalf of Minor/Ward, hereby release GUS and all legal representatives from any and all claims, demands, and liability relating to the Media, including all claims for invasion of privacy or right of publicity.

  3. I, individually, and/or on behalf of Minor/Ward, hereby consent and agree that such Media shall be the sole property of GUS and that GUS has the right to duplicate and reproduce the images as it may desire, free and clear of any claim on the part of myself, and/or Minor/Ward.

  4. I (the “Legal Guardian”) represent and agree that I have the legal capacity and authority to act for and on behalf of: (i) myself; (ii) my child under the age of eighteen (18) years (the “Minor”); and/or (iii) the person with a disability or who is a ward of the state/court (the “Ward”). This release shall be binding upon me and/or the Minor/Ward, and my or the Minor/Ward’s heirs, executors, representatives, next of kin, beneficiaries, administrators, successors and assigns. 

  5. I HAVE READ AND UNDERSTAND THE FOREGOING RELEASE AND ACCEPT AND AGREE TO ITS TERMS AND SIGN IT VOLUNTARILY.
